restricting ssh to scp and/or sftp only

Recently there has been some discussion on BLU about how to restrict
ssh to only scp.  John Abreau (I believe) commented that the
commercial ssh server had a restricted shell to do this.  Since
OpenSSH does not, I decided to write one.
