I'm with Jim on this one. I came up with thousands of Google results and
nothing in the first 100 or so addressed the issue. None of those links
was the link above.

As it turns out this link was very useful. The location of the cgi
scripts had changed as a part of cleaning up the directory structure and
the config file was providing basic authentication on a non-existent
directory. Once I fixed the config by correcting the path things went
back to normal and worked.
