  I actually haven't seen one yet that couldn't still happen (assuming
you had all the pieces, e.g., 20 year old VAX or whatever).  Might be
less likely due to experience, but then again, most of these mistakes
are of the "human error" variety, and they have yet to release a patch
for that.
