I use xfig.  It works great and you still have the unedited version of
the image when you're done.  Export to whatever format you want.

My use was to take a digital photo of a baby growth curve chart.  Then
I update the text and overlay splines showing current growth with xfig
and export a jpg for my sons web page.  The nice thing is I just add a
new pointto the spline eache time and export a new jpg, after each
doctors visit.  Sure makes the remote family members not need to ask
me for any information!

I've also done some screenshot and xray highlighting in the same maner
with xfig.
