Is this a DELL PERC by any chance?  I've seen similar (not exact) 
problems with them (the SCSI controller drops offline either 
temporarily or permanently) A combination of this particular BIOS 
firmware, that particular backplane firmware and a PERC firmware from 
September '04 (I'm making that up) made them go away.  All done in a 
certain order facing West, of course.

It had to do with too many fast small block writes at a time, which 
kernel 2.6/ext3 exacerbated, which I found by digging through the 
RedHat Bugzilla database.  If you need it I'll dig through my notes for 
you.
