VoIP <--> PSTN provider recommendations

I'm using Broadvoice as well, no complaints, but I don't really use it
all that much, also BYOD with a Sipura SPA-2000.

I've had VoicePulse in the past, and wasn't too impressed with them. The
call quality was fine, but has some issues with their billing system and
their web interface didn't feel very polished.
