I've been pretty happy with www.voipjet.com (for outgoing long distance
and international) and www.teliax.com (for remote phone numbers - call
origination, and long distance within NH - call termination). They both
support SIP and IAX - I use IAX given my nested firewall configuration.

Teliax has both "pay as you go" plans and monthly plans. voipjet has
just "pay as you go".

I'm looking into some others that provide international DIDs and have
sub-cent rates. But those all require me to sign NDAs.
